Emergencies, while rare, can occur, and knowing how to handle them is critical for maintaining client trust and professional credibility. Here’s a practical guide for navigating potential complications during filler procedures.
Recognizing Allergic Reactions
An allergic reaction can present as urticaria (hives), airway compromise, swelling, or shortness of breath. Immediate actions include:
Mild Reactions:Â Administer Benadryl 50 mg IM or orally, and consider 60 mg prednisone by mouth to reduce symptoms.
Severe Reactions:Â For airway compromise, administer an Epipen and call 911 immediately. Ensure the medical rescue box in your treatment room is stocked and easily accessible.
Managing Medical Emergencies
Signs like chest pain, weakness, confusion, or shortness of breath require prompt attention:
Lay the client on a bed.
Check their pulse and oxygen levels using pulse oximeters available in your treatment room.
Call 911 and provide accurate information to emergency responders.
Vasovagal Syncope: Stay Prepared
This fainting episode often results from fear, pain, or the injection itself. Respond by:
Laying the client on a bed and lowering their head.
Using ice packs and, if needed, elevating the legs.
Avoiding unnecessary movement, such as walking to the bathroom, to prevent falls.
Addressing Significant Bleeding
Bleeding can be a natural part of procedures, but if it becomes significant:
Apply pressure to the area for 1-5 minutes.
For brisk or concerning bleeding, consult your medical director and consider contacting 911.
Arterial Compromise: Act Quickly
Arterial compromise from fillers is a rare but critical situation. Steps include:
Immediately injecting 1-4 vials of Hylenex (without dilution).
Considering the use of chewable aspirin.
Applying warm packs to the area.
Documenting the incident with photos and notifying Allergan or Galderma, your malpractice provider, and your medical director.

How to initiate treatment.
